Programs/Living Services

Support For the Ones You Love!

We provide an extensive array of support services delivered directly to the consumer's home, workplace, and community. These services are typically funded through the Innovations Waiver Services program, as well as state, local, or private funding.

Alternative Family Living

A flexible partnership or an Alternative Family Living (AFL) setting of their choice, allows them to be an active participant in the home and in the community. This service includes direct assistance with activities of daily living, household support essential to the health and safety of the individual. Support with appointments, interpersonal and social skills that enables the individual to live as independent as possible in the home and in the community.

Residential Living

Level 1- Level 2 -Level 3/ -Level 4 – provides individualized services and support to enable a person to live successfully in a Residential Setting/Group Home. The intended outcome of this service is to increase or maintain the person’s life skills, provide supervision, maximize his/her self-sufficiently, increase self determination, and ensure an opportunity to be involved in his/her community.

Supported Living

Level 1/Level 2/Level 3/Level 4 - A flexible partnership that enables a person to live in their own home, with supervision, support and responsibility of the person and agency. This service includes direct assistance as needed with activities of daily living, household chores essentials to health and safety. Allowing for individual budget management, attending appointments, and interpersonal/social skills, to enable the individual to live in a home in the community.

Day Support Programs


Our Day Support Programs are an individual/group service that provides assistance to participants with acquisition, retention, or improvement in self-help, socialization and adaptive skills. These programs are furnished in a non-residential setting, separate from the home of facility, where the participant resides. Day Support focuses on enabling the participant to attain or maintain his or her maximum functional level and is coordinated with physical, occupational, or speech therapies listed in the individual's support plan. Transportation to and from the participant’s home and in the community is included.

Supported Employment


Supported Employment provides assistance with choosing, acquiring, and maintaining a job for participants ages 16 and older, for whom competitive employment has not been achieved, and/or has been interrupted or intermittent. Pre-job training/education and development activities to prepare a participants to engage in meaningful work-related activities which may include career/job shadowing, assistance in training, preparation, mock job interview skills, and assistance with job retention.

Respite 


Respite Services provide periodic support and relief to the primary caregiver(s) from the responsibility of caring for the participant. This service enables the primary caregiver to meet or participate in planned or emergency events, and to have planned time for him/herself and/or family members. Respite may include in and out-of-home services, inclusive of overnight, weekend care, emergency care (family emergency based or at home crisis).

Community Living & Support


Community Living & Support is an individualized service that enables the waiver beneficiary to live successfully in his/her own home, the home of his/her family or natural supports, and be an active member of his/her community.
